Providing Care at Home Within 48 Hours

Today, Tuesday 14th July, we can announce the launch of our Rapid Personalised Care Service.  Fully funded by the NHS Herts Valleys Clinical Commissioning Group, the service delivers packages of care to patients (in South West Hertfordshire) in their homes within 48 hours of referral to the service.

The service is for those in the last few weeks of life and it is designed to provide personalised care for a period of up to 12 weeks. Healthcare professionals can refer to the service.  Patients’ needs are assessed and an individualised plan of care put in place to enable them to stay safe and comfortable in their home, with visits from Healthcare Assistants up to four times a day.  In the first week of the service the team carried out 692 visits to patients in their homes.

Liz Kennedy, Community Services Manager, said: “Beginning a package of care within just 48 hours of referral will have a significant impact for patients who are in their last weeks of life, and means that as a community team we are able to provide more support and care out in the community and help reduce unnecessary hospital visits.  We know people want to stay in their own homes at end of life and we are pleased we can help them and their families in achieving that.”
