Social Worker

We are looking for a dynamic, innovative and highly organised social worker to both advise our Community Services and work in our Inpatient Unit with the main emphasis being discharge planning from admission where appropriate.

You will provide practical, social and emotional support to patients and their families and be proactive in the discharge planning process (working with statutory providers of care).  Together with the other Social Worker, you will be the Safeguarding and Carers Champion for the organisation, providing annual mandatory Safeguarding training for clinical staff and leading on relevant policies and guidance regarding these topics.

As part of the wider multidisciplinary team you will contribute to the provision of a high standard of holistic care.  Alongside this you will support Quality and Audit within the Hospice.

You will:

  • Be a qualified Social Worker with up to date professional and technical expertise
  • Be HCPC registered
  • Have excellent communication and organisational skills
  • Be able to work as part of a multi-disciplinary team
  • Understand ethnic, cultural and religious influences on patients and their families and how these affect (or are affected) by life threatening illness or bereavement
