Senior Social Worker

We have an opportunity for an enthusiastic, experienced and compassionate Senior Social Worker to join our multidisciplinary team at Peace Hospice Care located in Watford.

This post is 22.5 hours and you will be responsible for the social worker team in the Inpatient Unit, overseeing discharge planning and the provision of practical, social and emotional support services for patients and their families.

In addition, the post holder will also act as a resource person for all members of the MDT in relation to psychosocial aspects of patient and family care, providing social work support and advice to the Community teams and Outpatient Services.

Experience of participation in the implementation, audit and review of policies procedures and systems for the delivery of an effective specialist palliative care service is desirable, as is experience of leading on / contributing to the development of service improvement initiatives across services.

The ideal candidate will have excellent communication and organisational skills and the ability to work as part of a multi-disciplinary team. As part of the wider multidisciplinary team you will contribute to the provision of a high standard of holistic care.
