Rehabilitation and Wellbeing Team Lead

We are looking for an experienced Allied Health Professional team leader from any AHP profession to join our outpatient and inpatient teams at Peace Hospice Care. You will be responsible for the coordination and day to day operational management of the rehabilitation and wellbeing services across outpatient, inpatient and outreach services.

You should be educated to degree level and will have excellent staff management skills in a multi-professional setting. A key part of this role is to line manage a team made up of physiotherapists, social workers, a creative arts wellbeing coordinator and two assistant practitioners. You will work closely with other PHC teams, including the Counselling and Bereavement Team and Compassionate Neighbours. Experience of building collaborations, partnerships and networks will also be vital as our outpatient services continue to extend out into the local community.

You will be expected to maintain and develop your clinical skills as part of your role, ensuring patients and their families receive timely and effective holistic care that meets their changing needs. Some knowledge of medical devices will also be desirable as you will work towards becoming the Medical Devices Safety Officer responsible for clinical equipment across all services, alongside the Facilities Team.

Peace Hospice Care community and outpatient services works across the South West Hertfordshire locality, covering 36 GP practices which serve a population of approximately 320,000.

You will be a core member of the integrated Senior Clinical Management Team, and will be required to demonstrate effective leadership abilities to proactively deliver upon the clinical services and wider organisational strategic objectives. Your outstanding character must align with our organisational values and you will be required to demonstrate that you are passionate about making a difference and can be an advocate for our patients, families and the population we serve.

Our CQC rating is ‘outstanding’ for care with an overall ‘good’. We would expect you to have experience of leading a team that demonstrate Effective, Responsive, Safe, Caring, Well-led and evidence-based care.
