Rapid Personalised Care Service Team Lead

Are you ready for your next challenge – to lead a new, dynamic, team of health care assistants delivering end of life care in patients’ homes?

Do you have what it takes to clinically lead this new team and ensure delivery of a high quality, responsive, service across South West Herts?   You will be supported by a team coordinator who will work alongside you to help develop health care assistants’ skills, knowledge and practice.

If so, we have the ideal position for you.  We are seeking an inspiring, motivated, Senior Nurse as a Team Lead for our Rapid Personalised Care Service (RPCS).

You will demonstrate clinical excellence/knowledge by working as an expert clinical practitioner in palliative care and you’ll share that knowledge and expertise when working alongside staff, supporting them in developing their skills, knowledge and practice.

Always looking for opportunities for service development, you’ll understand the importance of monitoring quality of care through audit/spot checks.

This exciting new post will lead on, and be responsible for, the recruitment and service development, training, education and management of the Rapid Personalised Care Service in South West Hertfordshire, supported by a Peace Hospice Care RPCS Care Co-ordinator.

We completed a successful pilot with our partner hospices, delivering supportive care in the last 12 weeks at home for patients at end of life.  Now, we need a Senior Nurse who can take this work to the next level, helping to build and manage a new and dedicated team.
