If you’ve answered ‘yes’ to any of these questions then we would love to hear from you.  We are seeking an organised, determined and motivated Care Co-ordinator for our Rapid Personalised Care Service (RPCS).

In this exciting new post you will report into the Team Lead and work alongside staff, supporting them in developing their skills, knowledge, education and practice as part of the Rapid Personalised Care Service in South West Hertfordshire.

You will be the point of contact for patients, carers, families and internal and external healthcare professionals and will be in a key role, receiving referrals for home care support and allocating care visits for carers in the team.

Effective monitoring and review sits at the heart of this role.  You will ensure that initial care plans are set prior to review by Service Lead and that every patient has a formal review within the specified timeframe.

 

You will be:

  • Highly organised with good planning and co-ordination skills
  • Able to think on your feet with new cases and changing demands.
  • Experienced in rostering staff in a geographical area alongside frequency of patient visits.
  • Competent in using IT with experience of accurate record keeping and data completion

Your excellent interpersonal skills will enable you to work and communicate with people across our community.

 

The RPCS service is based at Peace Hospice Care in Watford Town Centre and operates a 7-day service alongside our Hospice at Home Nurse response team.  The Co-ordinator role will be required to work Monday – Friday but some flexibility will be needed to support the service and its requirements.

We completed a successful pilot with our partner hospices, delivering supportive care in the last 12 weeks at home for patients at end of life.  Now, we need a Care Co-ordinator who can work with our Team Lead to take this work to the next level – to help support a new and dedicated team and to co-ordinate care rotas, plans and visits.
