Rapid Personalised Care Service Carer/Healthcare Assistant

If you have answered ‘yes’ to any of these questions then we would love to hear from you.   We are seeking caring, compassionate and organised individuals to work as Carers for our Rapid Personalised Care Service (RPCS).  Full training will be provided.

You will work in the community, in a key role delivering personalised care and support for patients identified as requiring care and support at the end of life.

You will work alongside the patient’s family and/or health care professionals, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with our Peace Hospice Care vision, values and objectives.

We are looking for calm, confident and sensible applicants, who are passionate about caring for patients.  You may sometimes be visiting patients as a lone carer, so should be happy to work in this way as well as with other carers in your team.  Full training is provided together with a robust competency and induction programme and you will be fully supported by the RPCS Care Coordinator and the Team Lead.

More than providing care, you will be an essential source of support and advice for patients and families when they need us.

As this role is delivers services in the community it is essential that you are a car driver and have access to vehicle for use.

The RPCS service is based at Peace Hospice Care in Watford Town Centre.  It will operate across 7 days, and you will be work a range of shifts to cover the service from 7am -11pm, alongside the nurses in our Palliative Response Team.  This role attracts enhancements for unsociable hours and full and part-time positions are available.

We completed a successful pilot with our partner hospices, delivering supportive care in the last 12 weeks at home for patients at end of life.  Now, we are recruiting individuals who can work as part of a new, dedicated team to help take this work to the next level, supporting the community in South West Hertfordshire.
