Outpatient Team Lead

We are looking for a dynamic, pro-active, reliable and highly organised Team Lead to manage part of our Outpatient Service, alongside another Outpatient Team Lead (already in post).

This is a fixed term contract until 31st January 2021.  However, this will be reviewed periodically and may be extended.

This role is Part-time – 22.5 hours required over a minimum of 3 days (0.6 Full Time Equivalent)

 

Your main emphasis will be leading the team covering Compassionate Communities, Outreach and the Bereavement and Counselling service and managing the relevant coordinators. This is one half of the Outpatient provision which provides practical, social and emotional support to patients and their families in order to maintain the best quality of life possible while living with a life limiting illness, or through bereavement.

You will be a quality champion for the organisation, alongside the other Outpatient Team Lead, focusing on a specific topic for example; nutrition, safeguarding, infection prevention and will provide annual mandatory training for clinical staff and lead on relevant policies and guidance regarding these topics.

As part of the wider multidisciplinary team you will contribute to the provision of a high standard of holistic care.  Alongside this you will support Quality and Audit within the wider Hospice.

  • With a Health or Social Care-related degree, or evidence of working at this level, you will have experience of working with long term conditions/palliative and/or within bereavement care.
  • An excellent communicator who is confident in presenting to senior teams and external professionals
  • Excellent organisational skills and the ability both to work independently and as part of a multi-disciplinary team.
  • Knowledge of relevant legislation and best practice relating to safeguarding patient safety, human rights, mental capacity and the Children’s Act
  • Ability to lead change and motivate team working
