The West Herts Palliative Care Referral Centre (PCRC) was awarded the Innovation in Care award at the national Hospice UK Awards 2017 in Liverpool in November

The PCRC is a joint partnership between Peace Hospice Care, The Hospice of St Francis, Rennie Grove Hospice Care and Hertfordshire Community NHS Trust.

Jackie Tritton, Director of Patient Services, and Lisa Goodman, Community Services Manager, both from Peace Hospice Care accepted the award on behalf of the partnership. 

Speaking about the award, Jackie Tritton said: "The Palliative Care Referral Centre is a great example of local health organisations sharing their skills, resources, talent and expertise in a way that helps their local communities by making it quicker and easier for health and care professionals to make referrals.  I'm delighted to see our partnership working, and the impact it's making, recognised with such a prestigious national award.

The Palliative Care Referral Centre, which launched in March 2017, was commissioned by the Herts Valleys Clinical Commissioning Group.  It provides a single point of referral for palliative and end of life care patients registered with a Herts Valleys Clinical Commissioning Group GP.  It reduces duplication, makes referrals to palliative and end of life care services quicker and easier for health and care professionals and, also, makes better use of commissioned resources.

Previously, referrals were received by each of the partner organisations individually. Referrals are now received by the Palliative Care Referral Centre and then directed to the most appropriate service to meet the patient's need.
