We are looking for a qualified Social Worker (Band E).
Based in our InPatient Unit, Watford
Peace Hospice Band E (£26,100 full time equivalent to be pro-rata)
Part-time - 25 hours required over a minimum of 4 days
We are looking for a dynamic, innovative and highly organised Social Worker to predominantly work in our InPatient Unit, at the Hospice in Watford, with the main emphasis being discharge planning from admission, where appropriate. You will provide practical and social support to inpatients and their families and be proactive in the discharge planning process (working with statutory providers of care). You will also be the Safeguarding Champion for the organisation, providing annual mandatory Safeguarding training for clinical staff and leading on relevant policies and guidance regarding these topics.
The ideal candidate will have excellent communication and organisational skills and the ability both to work independently and as part of a multi-disciplinary team. As part of the wider multi-disciplinary team you will contribute to the provision of a high standard of holistic care. Alongside this you will support Quality and Audit within the wider Hospice.
