Community Services

Our Community team works within the Community Palliative Care team providing specialist palliative care for people with a life-limiting illness at home in South West Herts seven days per week.

The service works as a multidisciplinary team (MDT) of health and care professionals and volunteers. They work closely and in partnership with GPs, specialist community nurses and other community services to ensure the best plan of care is in place. The Hospice at Home team help through:

  • Expert symptom management and psychological support as well as practical care and emotional support
  • Support in the prevention of avoidable and unwanted hospital admissions
  • Support after discharge home from hospital or hospice
  • Support and respite for families and carers

Compassionate Neighbours

Compassionate Neighbours is a social movement of local people enabled and supported to be more compassionate in their local communities. They provide social and emotional support to people towards the end of life due to age or illness. They also share their knowledge and experiences of end of life, death, dying and loss within their community and provoke engagement with these issues. 

Compassionate Neighbours can support people by: 

  • Visiting regularly
  • Offering friendship, emotional support and a listening ear
  • Helping them to do the things they like doing
  • Helping them stay connected to the community as well as family and friends

Palliative Response Team

We have rebranded our Hospice at  Home service, transforming the way the service works. The service will now operate under a new name: Palliative Response Team (PRT) and provides more responsive care with extended hours, allowing patients to stay at home at the end of life, if that is their wish.

Rapid Personalised Care Service

The Rapid Personalised Care Service (RPCS) provides specialist care so you can remain at home.

It is designed to offer personalised care for a period of up to 12 weeks to provide people with the daily support they need to remain safe and comfortable at home during their final weeks of life.
