Hospice at Home

Our Hospice at Home service works within the Community Palliative Care team providing specialist palliative care for people with a life-limiting illness at home in South West Herts seven days per week.

The service works as a multidisciplinary team (MDT) of health and care professionals and volunteers. They work closely and in partnership with GPs, specialist community nurses and other community services to ensure the best plan of care is in place. The Hospice at Home team help through:

  • Expert symptom management and psychological support as well as practical care and emotional support
  • Support in the prevention of avoidable and unwanted hospital admissions
  • Support after discharge home from hospital or hospice
  • Support and respite for families and carers

Herts Neighbours Volunteer Service

Herts Neighbours is a support service at home provided by Peace Hospice Care volunteers. They support patients, families, and carers through the provision of respite, practical and emotional support within their care setting.

Herts Neighbours Volunteer Enablement Service
Herts Neighbours Enablement volunteers support, and encourage, patients in practising exercises that support their enablement at home aiming to improve wellbeing, mobility, and breathlessness over a period of planned sessions.
