If you have been diagnosed with a life-limiting illness - for example: heart disease, chronic obstructive pulmonary disease (COPD), neurological conditions, cancer or other life-limiting conditions, then we can help you, all of our services are free.
We want to support you to manage your condition and achieve the best quality of life possible. We can do that through:

• Advance Care Planning
• Wellbeing Support
• Psychological support
• Fatigue management
• Medicine management
• Spiritual care
