About our services
We provide free, specialist, care for patients and their families facing a life-limiting illness in South West Hertfordshire and the wider area.
Our patients have a range of illnesses including: cancer, neurological conditions, heart failure, organ failure and chronic lung disease.
All our services involve patients and their families/carers in their
individualised care choices and work in partnership with other health and social care professionals.
We are a charity and rely on the generous support of individuals and organisations that donate money, fundraisers and those that donate to, and buy from, our charity Shops as well as our committed volunteers. We need over £6 million a year to deliver our free services. For every £1 of income we receive less than 32p comes from the NHS. Thank you for your support.

We care for patients through our:
Our Outpatient Services offer a wide range of outpatient groups and activities for patients in South West Hertfordshire and the wider area. It provides rehabilitative and wellbeing interventions and workshops, including counselling, social, spiritual and emotional wellbeing support, physiotherapy groups and creative activities to support people from the earliest stage of their illness.
We care for patients at home – helping them to stay at home and supporting them and their family, assessing their needs and helping them to manage their illness including providing symptom management. We also have a support service provided by Peace Hospice Care volunteers. They support patients, families, and carers through the provision of respite, practical, emotional support within their care setting.
We provide care at the Hospice in our Inpatient Unit, to help patients manage their symptoms and then return home afterwards. We provide end of life care at the Hospice too. This is our Inpatient Unit Service.



Referrals
Health and care professionals and other agencies can refer patients to our services. We also accept self referrals from individuals who we then assess. We will always ensure that patient/client consent has been obtained to share their information with other relevant healthcare professionals.
Referrals are made through The West Herts Palliative Care Referral Centre. This is a partnership between The Hospice of St Francis, Peace Hospice Care, Rennie Grove Hospice Care and Hertfordshire Community NHS Trust. See below for contact details.

This Advice Line is for patients and families already known to us, and for healthcare professionals. Call: 01923 335 356
The Advice Line provides specialist support and advice quickly and efficiently with calls managed by an experienced nursing team (based in an Inpatient Unit) who will liaise with on call doctors and consultants as required.
